PCQC has finalized the BAA/DUA, participant agreement, and Exhibit A. Please download those documents and review with your legal team. The steps for joining the registry are included below.

The process for joining the paid patient-level portion of the PCQC registry is outlined below (the free program-level portion will involve a different process. More information to follow soon).

Step 1: Connect

  • Enlist the support of your internal team (including EHR experts, local Champion, leadership, and staff)

Step 2: Participate

  • Download the PCQC contracts
  • Review with your legal team
  • Execute the contracts and return to This email address is being protected from spambots. You need JavaScript enabled to view it.

STEP 3: Integrate and Submit

  • Initiate data integration discussions with your team and ours to determine your options
  • Review data fields in data integration pre-work
  • Create and send a test data file
  • Designate and train an Authorized User at your site
  • Begin regular data submissions

STEP 4: Access and Improve

  • View your data within 48 hours of submission
  • Run detailed reports
  • Compare results with national benchmarks
  • Participate in the PCQC Registry Program community and share best practices
